Cement Leakage Into the Neural Foramen during Polymethylmethacrylate-Augmented Pedicle Screw Fixation

Abstract

|
Polymethylmethacrylate-augmented pedicle screw fixation can significantly increase the pullout strength of the pedicle screws. Therefore, this procedure is often performed and can be useful for the instrumentation of the osteoporotic spine. However, there is a risk of cement leakage during this procedure. We report a case of intraforaminal cement leakage resulting in neurologic deficits, which occurred after pedicle screw augmentation with polymethylmethacrylate. Because the patient developed neurologic deficits, we performed nerve root decompression immediately and removed the bone cement. On the basis of the findings of our study, we recommend that cement-augmented pedicle screw fixation should be performed carefully, especially in case of revision surgery for cortical bone disruption.
